Medical Care Coordination - Los Angeles, CA - October to Present
Identifies patients who have not received health care related to their HIV needs for seven months or more. Is
part of an integrated Clinical team which includes outpatient medical, behavioral health, and dental services.
Identifies potential and existing barriers that prevent clients from seeking and utilizing health services on a
continuous basis. Collaborates with other staff members to create a psychosocial plan for patients and family
members to resolve challenges and difficulties that interfere with patients receiving maximum benefits of
behavior change. Ensures that clients have access to and knowledge of appropriate community resources.
Reports relevant data based on client interaction in monthly and quarterly reports. Compiles accurate records
and information by the organizational policy and procedures and external regulatory guidelines. Consults with
senior staff regarding additional clarification and for guidance in order to expedite all aspects of the work.
Promotes professionalism, and the highest quality of possible care to patients and their families. Conducts
timely follow up on participant's request, staff referrals, and recommendations. Travels locally to exercise the
aforementioned duties. Participates in case conferences, and clinical staff meetings.

I supervised detained minors from diverse backgrounds and mental and physical calibers during visits and
appointments, outdoor recreation, court visits, etc. Some juveniles are developmentally delayed and suffer
from mental disorder that range from mild to severe. I interact with detained juveniles' ages to as well
as detention staffing other juvenile halls and residential treatment centers. I assist detained juveniles that
suffer from diverse emotional and mental health issues. I write and complete mental health referrals, physical
intervention reports, special incidents reports as incidents occur, charting on minor's behaviors, etc. daily.
I also teach minors how to perform daily tasks (e.g., how to make up their beds, proper grooming, coping
skills, etc.) I have currently been with the Los Angeles County Probation Department for five years and I have
grown both professionally and personally, because I have learned to communicate and work with various
age groups. Through communicative skills I maintain group control of a large group of minors and staff as
well. I have experience in being the shift leader and maintaining a harmonious and team led environment
for both minors and my coworkers. In addition to the aforementioned I conduct CPR and Standard First Aid
classes for probation staff on and off site (Central Juvenile Hall). Administrative Support that includes: Daily
charting on minors behavior via Microsoft Word, Report Writing via Microsoft Word, Al jones weekly behavioral
charts(Microsoft Excel), internet, email, etc. Maintained CPR course record via Microsoft Word and Excel.
