E-Results


Assisted the IT Department to provide education to Physicians & Staff on the e-Results Viewer (web-based program that provides deliverable patient results to be obtained by medical practices).


 


Discharge Handoff


Team leader, worked collaboratively with VPMA, Director of the Hospitalist service, and the Manager of Lakeland University to improve inpatient discharge handoff from Hospitalist to Primary Care Physician.


 


PECOS (CMS Provider Enrollment, Chain and Ownership System)


Identified the need for communication with referring and ordering physicians to understand the importance and enroll in PECOS. Working with Lakeland\'s Patient Accounts, Outpatient Services and Homecare to develop a system communication plan to all providers who order/refer services for Medicare patients and a monitoring system of orders/referrals to ensure providers are registered in PECOS.


 


Physician Needs Assessment


- performed and completed the Provider needs assessment for the system and community. Analyzed and determined the supply of physicians and physician extenders in Lakeland\'s service area, allocated FTE status based on field knowledge. This data is used to determine the number of providers needed in the system to support recruitment efforts.


 


Physician Data Initiative


Team leader, brought together key associates to create a process for consistent provider information data entry, decreased associates data entry access, determined what departments need updates on provider information and created a daily report of physician/provider information changes.


 


Transfer Facilitator


Team leader, coordinated monthly team meetings to constantly monitor the Direct Admission process and continuously look for ways improve the service.


 


Day Task Force: Global Education/Communication Process


Co-team leader with the Manager of Lakeland University, determined the need for improved communication with Physicians and Clinical providers. With the assistance of the day team and the IT department, the Clinical Communication board was created. This tool is a concise communication process for the organization to identify and roll out clinical initiatives utilizing technology as the primary source.
